Information Systems and the Program Bureau developed the Forms Library for the DEBS staff.

The on-line system eliminates the need for offices to order forms, and reduces the wait time to receive a new/revised form, since all new/revised forms will be posted the same day they become available. In addition, more space will be freed up in District Offices and Central Supply, since the need to stock hundreds of forms is no longer required.

Forms Not Available On-Line

The majority of the forms and/or NOAs used in DEBS will be posted on this website, with the exception of those that are printed in bulk (i.e., PUB Brochures) and used in various packets. Since some brochures are several pages in length with a different format, it is not feasible to post these on-line. Therefore, some brochures will not be available on-line and must be ordered following appropriate ordering procedures.

Location of Forms

It is recognized that forms of high usage, such as those that are used in Intake/Redetermination packets, must be readily available for staff to assemble the packets. For that reason, statement of facts (applications) and forms that are used in packets will also be available in hard copy so a supply may be kept in District Offices, as well as available on-line. Forms used in Intake/Redetermination packets must be ordered through the DEBS Forms Desk and cannot be printed in bulk from the On-Line Website at District Offices.

The DEBS Forms Website indicates “On-Line & Warehouse” in the “Location” column for those forms which are available both on-line and in shelf stock, as well as those that are listed in this section.

Unable to Locate a Form

If a form that staff previously used is no longer available on-line, it is to be assumed that the form is no longer available and is obsolete.

Note: District Office Liaisons may contact the Program Coordinator for their respective program to inquire on forms that are in question. If an Office Management Coordinator (OMC) believes a particular form should be made available in shelf stock, the OMC may contact the Program Bureau at (408) 755-7540 and discuss the reasons why it should be made available.
