DocuSign Forms

DocuSign provides electronic signature technology and Digital Transaction Management services for facilitating the electronic exchange of information in a secure environment. DocuSign's features include authentication services, user identity management, and workflow automation. Signatures processed by DocuSign are comparable to traditional signatures. All information sent through DocuSign is encrypted.

DEBS Staff may provide clients with the option of having forms sent through DocuSign to expedite processing. Templates are available in DocuSign and indicated on the DEBS form library, staff may manually upload a form that is not available on the DEBS library.

 

Do not include Confidential or Personal Identifiable Information (PII) in the Subject line or in the content of the email. Forms sent through DocuSign may include the last 4 digits of the SSN and the case number.
