Action Needed If... Then...

Add-A-Person (adult/new- born/child). Refer to CW chapter 9 and CF HB 23 and for policy specific information.

The client does not have the required forms/ verifications,
  • Provides the appropriate forms and list of the required verifications with a due date.
The client has the required forms/ verifications,
  • Collects the forms and verifications from the client
  • Reviews for completeness.
  • Makes copies of any required verifications.
  • Date stamps the paperwork.
  • Provides documents to Clerical, per office process, to scan into Imaging and auto-task creation.

Note: Add the child to Medi-Cal if requesting Immediate Need (IN).

Case Information Questions. Refer to CW HB 10 and CF HB 25 for policy specific information.

The program(s) is/are discontinued due to SAR 7 related issues,
  • Advises the client to complete their SAR 7 or a new application as appropriate.
The program(s) is/are discontinued due to any other reason and Good Cause exists,
  • Collects any necessary paperwork and provides to Clerical for Imaging and auto-Task creation, as appropriate
  • Documents on the CalSAWS Journal Detail page actions taken and the Good Cause determination.
The program(s) is/are discontinued and Good Cause does NOT exist,
  • Provides the client the necessary forms for reapplication and advises the client to submit the completed forms as soon as possible.
  • Documents on the CalSAWS Journal Detail page actions taken.
Change of Address/ Telephone Number/ email address The client reports new contact information,
  • Updates the new contact information in CalSAWS.
  • Documents on the CalSAWS Journal Detail page.
The client reports changes to the shelter expenses,
  • Date stamps the received paperwork
  • Submits the paperwork to be scanned.
  • Provides the client with the CW 2200 form, if the shelter cost is questionable.
  • Creates a Case Flag to follow up on the case.
  • Documents on the CalSAWS Journal Detail page.
Completing Forms  
  • Assists the client with the completion of forms, if needed.
  • Reviews the forms with the client and clarifies any questionable information.
  • Provides to Clerical for Imaging and auto-Task creation if appropriate.
  • Documents on the CalSAWS Journal Detail page.

Homeless Assistance (HA)

Temporary Shelter (TS)

Permanent Housing (PH)

The case is eligible for TS/ PH,
  • Provides the client with the application for TS/PH.
  • Collects all necessary verifications.
  • Process the TS/PH payment.
  • Documents on the CalSAWS Journal Detail page.
The case is not eligible for TS/PH,
  • Explains the reasons for ineligibility
  • Documents on the CalSAWS Journal Detail page.
Verification of Benefits (VOB) The client requests VOB,
  • Processes the VOB and provides it to the client waiting in the lobby.
  • Documents on the CalSAWS Journal Detail page.
